DESCRIPTION

Ceftriaxone is third generation cephalosporin antibiotic and has broad spectrum activity with high degree of stability to beta-lactamases.

Anti-Microbial Action:

Ceftriaxone inhibits cell wall synthesis resulting in antimicrobial action.Cefriaxone has greater activity against gram negative bacteria than first or second generation cephalosporin. It is also active against positive bacteria and for many of gram negative bacteria.

PHARMACOKINETICS.

Ceftriaxone demonstrates non-linear dose dependent pharmacokinetics because of its protein binding. About 85%-95% is bound to plasma protein depending on plasma convertration of Ceftriaxone. After intramuscular injection mean peak plasma concentrations of about 80ug per ml have been reported 2-3 hours after administration of 0.5 and 1 gm of Ceftriaxone respectively. the plasma half life of Ceftriaxone is not dependent on the dose and varies between 6 and 9 hours.ceftriaxone is widely distributed in body tissues and about 40-65% of a dose of Ceftriaxone is excreted unchanged in the urine by glomerular filtration.Renal clearance is 5-12 ml/min while 40-50%is excreted unchanged in the bile.

CONTRAINDICATION OF CEFTRIAXONE:

Ceftriaxone is contraindication in patients who are sensitive to cephalosporin anitibiotic.Care should be taken while administration to nursing mothers because low concentration of Ceftriaxone are excreted in breast milk.ceftriaxone should not given in pregnancy.

SIDE EFFECTS of Ceftriaxone

Side effects is mild and transient are mild and transient in nature when they happen they should be treated symptomatically. The most common side effects are gastrointestinal disturbance (losses stool,diorrhoea,nosia,vomiting,cutaneous reaction include maculopapular rash,prurities,urticaris,oedema,and erythema,multiforma,haematological reaction include anemia leucopenia,neutropenia,thrombocytopenia,eossinophillia,headache,dizziness,drug fever and transient elevation in liver function tests have been reported in a few cases.

Ceftriaxone تیسری نسل کا سیفالوسپورن اینٹی بائیوٹک ہے اور اس میں بیٹا لییکٹامیسس کے لیے اعلیٰ درجے کے استحکام کے ساتھ وسیع سپیکٹرم سرگرمی ہے۔

اینٹی مائکروبیل ایکشن:

Ceftriaxone خلیے کی دیوار کی ترکیب کو روکتا ہے جس کے نتیجے میں antimicrobial ایکشن ہوتا ہے۔ Cefriaxone کی پہلی یا دوسری نسل کے سیفالوسپورن کے مقابلے گرام منفی بیکٹیریا کے خلاف زیادہ سرگرمی ہوتی ہے۔ یہ مثبت بیکٹیریا اور بہت سے گرام منفی بیکٹیریا کے خلاف بھی سرگرم ہے۔

فارماکوکینیٹکس۔

Ceftriaxone پروٹین بائنڈنگ کی وجہ سے غیر لکیری خوراک پر منحصر دواسازی کا مظاہرہ کرتا ہے۔ Ceftriaxone کے پلازما کنورٹریشن کے لحاظ سے تقریباً 85%-95% پلازما پروٹین کا پابند ہے۔ انٹرماسکلر انجیکشن کے بعد یعنی تقریباً 80ug فی ملی لیٹر کی چوٹی کے پلازما ارتکاز کی اطلاع بالترتیب 0.5 اور 1 گرام Ceftriaxone کے 2-3 گھنٹے بعد ملی ہے۔ Ceftriaxone کی پلازما نصف زندگی خوراک پر منحصر نہیں ہے اور یہ 6 سے 9 گھنٹے کے درمیان مختلف ہوتی ہے۔ Ceftriaxone جسم کے بافتوں میں بڑے پیمانے پر تقسیم ہوتی ہے اور Ceftriaxone کی خوراک کا تقریباً 40-65% گلومیرولر فلٹریشن کے ذریعے پیشاب میں بغیر کسی تبدیلی کے خارج ہوتا ہے۔ رینل کلیئرنس 5-12 ملی لیٹر فی منٹ ہے جبکہ 40-50% صفرا میں بغیر تبدیلی کے خارج ہوتا ہے۔
